Q: If we're moving, what should we do?
A: If you are withdrawing from Hudson Middle School, please contact the Middle School Guidance Office at 330.653.1320 to request a student withdrawal form. Information regarding last day of attendance, new school name and address, reason for withdrawal, and a parent/guardian signature will be needed to complete the form. Once the form is completed and returned, staff will be notified and all records copied to send to the new school. On the student's last day of attendance we ask that he/she to visit the Guidance Office for an exiting grade form. This form accounts for the return of classroom/media materials, physical education locks, and grades at time of withdrawal. The exiting grade form must be returned to the Guidance Office at the end of the day with appropriate information filled out. A copy will be made so that the student may take it to the new school. If you are entering Hudson Middle School, please call Lois Aubrecht at 330.653.1419. Mrs. Aubrecht is the central registration secretary for Hudson City Schools. She will provide a registration packet and set an appointment for return of all necessary forms. In addition to the completed forms for central registration, please bring the following:
- Certified Birth Certificate (no photo copies will be accepted)
- Proof of Residency
- Custody Documents, if applicable
After you have an appointment set with Mrs. Aubrecht, please contact Monica Slopnick at 330.653.1320. Mrs. Slopnick will provide an additional HMS registration packet and schedule an appointment for the new student and a parent or guardian with a guidance counselor here at Hudson Middle School. In addition to the completed forms for HMS registration, please bring the following:
- Immunization Records
- Last Report Card
- Special Education Documents
- Health/Medication Information
Q: If we're applying to a private school, what should we do?
A: Chances are the private school will need a copy of the student's transcript, standardized results, and a current report card. Please provide HMS Guidance Office with a signed Transcript Release form and a stamped envelope addressed to the appropriate school. Many private schools supply these in their application packets. If the school your student is applying to does not, please contact Monica Slopnick at 330.653.1320. Mrs. Slopnick will provide the necessary release form. Information needed includes: name and address of school, material to be sent, and a parent/guardian signature. Please allow one week for the processing of this information. Once acceptance and enrollment has been determined, please notify HMS Guidance with your plans.
Q: What are the criteria for Honor Roll?
A: The Merit Honor Roll recognizes students who earn all A's and B's on their quarterly report card. Students receiving all A's on their quarterly report card are recognized on the Distinguished Honor Roll. All subjects are included in determining honor roll.
Q: If a student needs to speak with a guidance counselor, what should he/she do?
A: The guidance counselors are available to meet with the students each day. Because instruction is so vital to the learning process, it's important that the students do not miss class. For this reason, we ask that the students meet with their counselor during team support times, lunch, and before or after school. Students must have a pass when visiting the Guidance Office.
